Owner Shipper Statement
This statement can be used in lieu of a Certificate of Veterinary Inspection (a.k.a. a Health Certificate) for compliance with new federal regulations regarding identification and interstate shipment of livestock for slaughter (the animal disease traceability rule).

A Certificate of Veterinary Inspection (CVI) is an official document that has to be completed and endorsed by a federally accredited veterinarian.  Animals moved interstate are required to have official identification (tags are available to producers through the Division of Agriculture) and be accompanied by a CVI unless those animals are going directly to slaughter.  Animals that move interstate to slaughter still need official ID, but the Owner Shipper Statement attached can be used in lieu of a CVI.  The Owner Shipper Statement can be filled out by the producer (no need for a federally accredited veterinarian) and it will be collected by the livestock market, auction, or slaughterhouse where the animal is sold.
